Operation Ignite: Differences Between Claim Denials and Rejections

Before diving into the core session, this focused discussion will clarify the critical distinctions between claim denials and rejections—two terms often used interchangeably but with very different operational and financial implications. Attendees will learn how to identify the root causes of each, understand their impact on reimbursement timelines, and apply best practices for timely and accurate resolution.

  1. Describe the difference between Claim Rejection (not processed and no on file with the payor) and a Claim Denial (claim on file and processed with the payor but not paid due to coverage rules; claim error; or medical necessity issues).
  2. Discuss what are the most common reasons for each and how they impact your revenue stream, and the timeframes to correct each timely. 
  3. Explain the importance of working rejections and denials timely, updating processes and/or implementing new processes to drive down errors internally, and the need for reporting against each.

Operation Elevation: Understanding Payment Delays in Infusion Operations

Attendees will gain insight into the most common causes of payment delays in home and alternate site infusion operations. The session will explore where breakdowns most often occur—from authorization to claim submission to payer follow-up—and provide practical strategies to minimize these interruptions and improve overall revenue performance.

  1. Describe the most common operational and payor-related issues with obtaining payment on the initial claim submission. 
  2. Discuss how to trend and find where the breakdowns are happening most frequently: front end verification/ investigation, POS issues, coding, records, PA issues, follow up issues, to better understand what areas need to be addressed. 
  3. Explain how to apply strategies and best practices once these areas are identified.

Operation Ignite: Use of Metrics to Identify and Resolve Revenue Barriers

This follow-up session will demonstrate how to leverage key operational and financial metrics to pinpoint revenue barriers and track performance improvements. Participants will explore how data-driven dashboards and trend analysis can reveal underlying issues in billing flow, payer response, and collections—helping teams transition from reactive troubleshooting to proactive revenue management.

  1. Describe how to identify and pinpoint revenue barriers using metrics and reporting can be implemented to be able to resolve and remove revenue hurdles more effectively. 
  2. Discuss how to review what is working and what needs to be fixed and or added via action plans and new processes using data-driven dashboards to quickly find trends that are causing revenue delays.
  3. Explain which operational and payor metrics matter or influence your office the most and why they are important, where the breakdowns occur and how the data is revealing those patterns, along with how to apply the metrics to help make operational decisions.

My name is Laurieal Snyder, and I have the privilege of serving as the Revenue Cycle Operations Supervisor for OptiMed Health Partners. I bring more than 14 years of dedicated experience in the revenue cycle arena, with a career that has allowed me to build a broad and deep understanding of the operational, billing, and regulatory elements that shape our industry.

I began my professional journey in physician billing, where I developed a strong foundation in reimbursement processes, compliance standards, and workflow optimization. From there, I expanded my expertise into the DME world, gaining valuable insight into complex payer requirements.
In 2016, I transitioned into my first role within the home infusion space; a move that proved to be both professionally rewarding and personally inspiring. It was in home infusion that I discovered a true passion for revenue cycle management, navigating intricate reimbursement pathways, and helping bridge gaps from patient care to collection of revenue.
Today, in my role at OptiMed Health Partners, I oversee revenue cycle operations with a strong focus on process improvement, payer relations, staff development, and ensuring an exceptional experience for both our patients and provider partners. I am committed to advancing our mission through operational excellence and to contributing meaningfully to the growth and integrity of the home infusion industry.
