A new parenteral nutrition (PN) referral has been received: Has patient been stable for weeks at goal PN or did PN start yesterday with orders for discharge home today? Or, is the order for a home start PN?  Assessing your patient’s PN status upon referral will guide a safe and timely start of care plan. This session will provide clinicians with strategies for assessing upon referral, communicating with prescribing teams to help fill gaps, leading to a customized and safe PN start of care.

Despite experienced home infusion teams implementing custom parenteral nutrition (PN) patient care plans, pieces can be missing for therapy excellence: partnership and communication among infusion provider and prescriber teams can be tricky to navigate; assessing PN needs for various underlying disease states can prove to be complex. This session will explore techniques for enhancing partnerships among PN care teams and will examine customizing assessments for various PN patient disease states. Case examples will showcase strategies to help fill in missing pieces for strengthening PN patient care. 

  1. Describe partnering among care teams for managing PN patients. 
  2. Discuss common PN patient disease states and assessment considerations.
  3. Identify strategies for partnering and customizing care plans to strengthen PN patient care.

Clinical Edge: Filling Gaps: Practical Methods for Weaning Patients from Parenteral Nutrition

Were orders received to “wean PN, or stop PN” without detail or direction from the prescriber?  Are you wondering if your patient is truly ready to wean from PN, and if so, how to wean safely? This session will provide actionable strategies for safely weaning patients from PN.

  1. Describe how to identify when weaning PN is appropriate.
  2. Discuss strategies for safely weaning PN.
  3. Explain monitoring and minimizing complication risk during PN weaning.

Lisa Kinder is a registered dietitian, maintains certification as a nutrition support clinician, and has worked in the home infusion setting since 2004. She is a clinical nutrition program manager at Optum Infusion Pharmacy. Her current role focuses on training and education programs for clinicians providing home parenteral nutrition. She is passionate in the areas of safe parenteral nutrition initiation in the home setting and the prevention of refeeding syndrome. Lisa has co-authored publications including abstracts, continuing education articles, a white paper, and has presented posters and educational sessions at national conferences including ASPEN and NHIA. She serves on NHIA’s Education Committee. Lisa received UnitedHealth Group’s Sages of Clinical Services Award in 2019, is a member of the home nutrition support team that was honored with ASPEN’s Clinical Nutrition Team of Distinction Award in 2022, received ASPEN’s HPEN Trailblazer Award in 2023 and became a NHIA Fellow in 2025.
