In the home and alternate site care settings, the perception of high-risk patients often centers on the complexity of the drug or therapy being administered, such as parenteral nutrition (PN) or pain management. However, the growing complexity of patients in these settings necessitates a broader view of risk factors, including co-morbidities, psychosocial challenges, and mental health conditions. Despite the critical need, many healthcare providers lack a comprehensive approach to identifying and managing the full spectrum of risks that place patients in high-risk categories. This gap in knowledge and practice can result in inadequate care planning, disparities in access, and poorer outcomes for vulnerable populations. By addressing this gap, healthcare teams can adopt multidisciplinary strategies to assess and manage high-risk patients more effectively, ensuring equitable access to care and improving patient outcomes.
